Diabetes Prevention Program

The National Diabetes Prevention Program (DPP) is a CDC-recognized lifestyle change program working to delay or prevent type 2 diabetes.  

Key Components of the DPP

  • CDC-approved curriculum with lessons, handouts, and other resources to help you make healthy changes.

  • A lifestyle coach, specially trained to lead the program, to help you learn new skills, encourage you to set and meet goals, and keep you motivated. The coach will also facilitate discussions and help make the program fun and engaging.

  • A support group of people with similar goals and challenges. Together, you can share ideas, celebrate successes, and work to overcome obstacles.

Am I Eligible to Participate? 

Program Eligibility: ​

  • Can not be diagnosed with Type 1 or Type 2 diabetes

  • Must be at least 18 years or older

  • Must have a BMI greater than 24

  • Not be pregnant

  • One or more of the following:

    • Fasting glucose of 100-125 mg/dl

    • Plasma glucose measured 2 hours after a 75 gm glucose load of 140 – 199 mg/dl

    • A1C of 5.7 – 6.4

    • Clinically diagnosed with gestational diabetes mellitus (GDM) during a pervious pregnancy (may be self reported)

    • A health care professional may refer potential participants to the program, but a referral is not required for participation in the CDC-recognized programs

    • OR a positive screening for prediabetes based on the CDC Prediabetes Screening Test (https://www.cdc.gov/prediabetes/takethetest/) or a screening result indicating high risk for type 2 diabetes on the hard copy or electronic version of the American Diabetes Association Type 2 Diabetes Risk Test (http://www.diabetes.org/risk-test

Time Commitment & Cost 

The program runs for one year. 

  • During the first 6 months of the program, you will meet about once a week.

  • During the second 6 months, you’ll meet once or twice a month.

  • Making lifestyle changes is an ongoing process. Staying in the program for the full year is essential to help you stick to new habits and avoid slipping back into old habits.

All members covered by EMI health insurance are eligible to enroll with no additional cost.

  • Cohorts will run every 3 months and are based on a first come first serve basis. Enrollment is limited.
